Active All Day but Still in Pain? Here’s What May Be Going On

You’ve spent the day lifting, carrying, walking or standing. By the time you get home, your back aches, your shoulders feel heavy and you’re thoroughly worn out.
We’re often told that being active is good for our health, so it’s reasonable to ask: “If I’ve been moving for hours, why am I still in pain?”
That apparent contradiction lies at the heart of what researchers call the physical activity paradox.
In a nutshell
Physical activity at work and activity during leisure time aren’t always interchangeable.
Activity at work may be prolonged, repetitive, externally paced and difficult to vary. Leisure-time activity is often shorter, more varied and easier to adjust: you may be able to change the pace, reduce the intensity or stop when necessary.
Research has found a contrasting pattern in persistent musculoskeletal pain. Higher leisure-time activity has been associated with a lower likelihood of persistent pain, while higher occupational activity has been associated with a greater likelihood of persistent pain and pain affecting more areas of the body.
This pattern is known as the physical activity paradox.
It doesn’t prove that work caused someone’s pain or that leisure-time activity prevented it. It doesn’t mean desk work is healthier, and it certainly doesn’t mean every manual worker should add another workout after an exhausting shift.
Instead, it suggests that the total amount of movement tells us only part of the story. Its intensity, duration, repetition, variety, level of control and the opportunity to recover may all influence the demands it places on a person.

Why a busy job can feel different from exercise
A controlled set of ten lifts followed by a rest isn’t the same as hundreds of lighter lifts spread across an eight-hour shift.
Physical jobs commonly involve:
repeated movements;
prolonged standing or walking;
lifting, pushing or pulling;
working overhead or in confined positions;
limited opportunities to change task; and
several demanding shifts in succession.
Planned exercise is normally intended to improve a particular ability, such as strength or cardiovascular fitness. Work activity is intended to get the job done.
Your job may develop useful task-specific strength and endurance, but it might not provide balanced fitness. It may repeatedly load one body area while leaving other abilities undertrained.
There’s also the question of control. You can pause a walk or reduce a gym weight. Stopping halfway through a shift because your shoulder feels tired may be rather less practical.
What the research really tells us
A 2025 study involving 2,787 adults found that higher leisure-time activity was associated with lower odds of persistent pain and fewer painful areas.
Higher occupational activity showed the opposite pattern: it was associated with greater odds of persistent pain and more painful areas.
That’s interesting, but it doesn’t prove that leisure exercise prevented pain or that work caused it. The researchers measured pain and self-reported activity at approximately the same time.
Other explanations remain possible. Pain may make work feel more demanding, for example, or discourage someone from exercising during their free time. Sleep, stress, general health and working conditions may influence both.
The sensible message is therefore not “work is bad”. It’s that advice about exercise should take someone’s existing workload and recovery into account.
What can you do with this information?
Start by looking at your whole week rather than treating work and exercise separately.
Notice which tasks or parts of the shift aggravate symptoms.
Look for pain that accumulates across consecutive working days.
Consider whether the task, weight, repetition or working position could be changed.
Choose exercise that complements your job rather than copying it.
Begin with an amount you can recover from and review how you feel the following day.
Put harder exercise on lighter working days where possible.
Someone who walks and carries throughout the day may benefit from a brief, controlled strength programme rather than simply adding more steps. Another person may first need fewer workload peaks and better recovery before progressing exercise.
Sleep, food, fluids and rest don’t sound particularly exciting, but they affect how much physical demand you can tolerate. A very hard shift may already be enough load for that day.
Practical takeaway: The aim isn’t always to do more. It’s to create a better balance between occupational load, useful exercise and recovery.
When is an assessment worthwhile?
Consider seeking professional advice if pain:
keeps returning or is gradually worsening;
affects your work, sleep or normal activities;
spreads or becomes more widespread;
is accompanied by recurring numbness or weakness; or
isn’t improving with sensible changes to activity.
Seek urgent medical help for new bladder or bowel problems, numbness around the groin or inner thighs, rapidly worsening weakness, significant trauma, chest pain or severe pain accompanied by fever or feeling very unwell.
